Who can refer

Clinicians working within secondary care pain settings in Scotland, can refer suitable patients, by completing the electronic referral form and emailing it to Scottish.PMP@ggc.scot.nhs.uk.

 

Considering making a referral

It is anticipated that the residential in-person programme will be suitable for most patients. The development of the virtual programme has provided the opportunity to meet the needs of people unable to access a residential programme. The specialist interdisciplinary assessment will determine which programme best meets the individuals’ needs based on referrers expert opinion, team clinical judgement and patient choice.

Making a referral

Please provide the Pre-Referral Patient Information Leaflet to the individual at the time you make the referral.

If the referral form displays as misaligned then you can fix the problem by using this guide  

Referrals that are completed electronically and emailed as an attachment to Scottish.PMP@ggc.scot.nhs.uk are preferable. Alternatively, completed forms can be printed, then completed by hand and sent in the post. Address can be found on the bottom of the referral form.
